If a notice appears when activating a tab, follow the instructions shown in that pop-up itself: opening the right tab and clicking the icon again, or retrying. The browser's own internal pages can never be captured, by Chrome's design.

Chrome only authorizes audio capture from the tab that's active at the moment you invoke the extension. Open that tab first and click the icon again.
No. It's only needed if you want to see your output devices' real names. Without it, the extension works normally using the system's default output.
Equalizer profiles, global volume, and general settings are saved permanently. The audio output chosen for each tab is session-based and is lost when you close that tab or close Chrome entirely.
Yes. If you turn on the optional sync in Options, your global volume, auto leveler, loudness normalization, language, and custom profiles travel with you between computers using your own Chrome account.
